ATA Application Forms
Here are downloadable versions of ATA application forms that we use in Lago Vista ATA Black Belt Academy.
You may print the applicable forms and use them for your next ATA event, e.g. a rank test.
All forms are in PDF format. To use them, you need Adobe PDF Reader installed on your computer. It is usually installed on most computers, but if you don't have it installed on your computer, you can download a free copy from Adobe.
Graduation Forms
For color belt graduations at our school, you will need to submit a Graduation Registration & Letter Of Intent To Promote.
